Due to allegations of a large number of hard disk drives arriving dead or dying within short periods of time, and as was suggested by Alan Cox on the linux-kernel mailing list, this page will attempt to collect data on this matter.

Please enter your dead drive information below. If your disk was dead on arrival, then just put the same date in both places. Also, please don't use commas in any of the fields ;).
